The Health Information Service manages the patient data flowing in and out of the WCVM Veterinary Medical Centre, helping to tell your animal's health story. 

Privacy and confidentiality

Just like human health records, protecting your animal's health information is essential. That's why the WCVM Veterinary Medical Centre requires your consent to send records to anyone who is not listed as an owner or a veterinarian on your animal's chart.

Requesting patient records

Your clinician will send you a copy of the discharge notes containing the story of care (such as medical history, physical exam and other notes) during your animal's visit to the VMC.

We are open from 8 a.m. to 4:30 p.m., Monday to Friday. Please provide us with as much lead time as possible to process your request. 

While most requests are completed within 72 hours, please note that we prioritize all requests to patients in distress at another veterinary clinic.
